For Dean's birthday John agrees to take him along for a hunt. After Sam disobeys his father and leaves the car, Dean is attacked by the creature they are hunting.
He is rushed to hospital and the guilt eats away at John, who also blames Sam for what happened.
Dean fights for his life and a familiar face acts as a guide helping Dean find his way back home.
